SWIMMING POOL LIABILITY WAIVER (STUDENT)

This form is to be filled out by all students wishing to utilise the Sayfol swimming pool for
swimming lessons, CCA and recreational swimming.

1) I understand that there is NO LIFEGUARD ON DUTY (a lifeguard is required only if the depth
of the pool is above 1.5 meters) and that there ought to be a minimum of two swimmers
present when using the pool at all times.

2) I agree to familiarise myself/ my child with Sayfol's rules governing the swimming pool
and agree to abide by them. I understand that Sayfol International School retains the right
to suspend or expel students if they fail to comply with the pool rules (applies to swimming
pool, changing facilities or the surrounding area).

3) The school, to the extent permitted by law, shall not be liable or responsible for all or any
loss of life or injury, of any nature, incurred by individuals in relation to or arising from the
utilization of the Pool Amenities.

4) I understand and acknowledge that there are certain dangers and risks of physical
injury while swimming and using the Pool Amenities. I also understand that in order to be
allowed to use the Pool Amenities I must waive my rights to hold the school liable for any
injury or damage which I, my family members, or my guests may suffer while utilising the
Pool Amenities. I voluntarily agree to assume the full risk of any and all injuries, damages
or loss that I, my family members, or my guests may sustain as a result of using the Pool
Amenities. I further agree to waive and relinquish all claims against the School that I, my
family members, or my guests may have as a result of using the Pool Amenities.

This form must be signed by a Parent or Guardian of children (under 18) who intend to use
the pool. Visitors over the age of 18 will need to complete their own form. Sayfol shall
recognise a digital signature to be a legally binding signature.
