Indirect Pay:

1. Health Benefits Plan: premiums are 50% company paid and 50% employee paid. The plan includes:

 $300 per 2 calendar years for corrective lenses

• Medical services and supplies, e.g., orthotics ($400 per 3 calendar years), hearing aids ($500 per

• Vacation: all employees start with 15 days of annual vacation; increases to 20 days of vacation
