Dear Ma’am:
This is in connection with the Final Notice to Liquidate under Memorandum No. 08-17-2023(01)
dated August 17,2023 which was issued by the University President. The memorandum states that
you are required to liquidate your Cash Advance and submit the Liquidation Report to the Accounting
Office on or before August 31, 2023.
In line with this, you are hereby REQUIRED to submit the Liquidation Report together with its
supporting documents on or before November 22, 2023. Failure to liquidate on the said date will be
subject to sanctions provided under Item 1 of Section 122 of PD 1445, which states that failure on the
part of the officials concerned to submit the relevant documents and reports shall automatically cause
the suspension of payment of salaries until they shall have complied with the requirements of the
COA. In your case, your salary suspension shall commence on December 2023 until you have
submitted the required Liquidation Report to our Office.
We are expecting your preferential and prompt action on this matter.
