Response to 053 Unemployment Questions

1) I resigned from work for two reasons.

-First, to provide care for my husband. My husband’s medical condition changed
requiring care. Do to working graveyard shift and the commute I was hardly home and
was unable to provide care.
-Second, fear of contacting Covid and exposing my husband to it. Covid 19 exposure
is high. Co-workers have contracted it and I have been sent home due to exposure to
coworkers with positive testing for Covid 19.

2) I provide care Monday thru Sunday

3) 6pm - 8am

4) cook meals, medications, dressing, doctors appointments, encouragement, and

support

5) No, not for evenings

6) Yes, from 8am - 6pm

7) N/A

8) N/A

Attention: Sarah Cantu,

The department received your response regarding the issue about your able and
available for work. We have additional questions regarding this issue. If a
question is not applicable, then notate the question with a N/A. Please make every
effort to respond to each question during the initial response. A second request
only causes unnecessary delays in the processing of this claim. We will use this
information to decide the eligibility for unemployment insurance benefits.

1 You previously reported that you had to resign from your job to provide
care for your husband. You currently reported that you have been able and available
for at least 40 hours each week since 7/17/22. If you were able and available for
40 hours of work each week, why did you have to resign from your job?
2 What days do you provide care to your husband?
3 What hours of those days do you provide care to your husband?
4 What specific type of care do you provide?
5 Is there anyone else who can provide this care while you look for work
or if you become employed?
6 You indicated that you were able and available for work in the
occupation of waiter and waitresses. Are you still available to do this type of
work?
7 If not, what type of work are you available to do?
8 Do you have experience in this type of work? If not, what makes you
suitable for this type of work?

A response is needed no later than 8/17/22 by 11:45 AM Pacific Time, otherwise a

decision will be made based on the available information. (For security and
internal purposes, please do not change the subject line of this email or include
your SSN in your reply.)
Sincerely,
