RE: Request for payment of November 2011-January 2019

delinquencies through installment

SIR/MA’AM:

We are writing this letter to respectfully request from your good office to allow
us to pay our pending delinquencies from November 2011 to January 2019 through
installment under the Pandemic Relief Restructuring Program -3. For this purpose, we
intend to follow the schedule of payment provided under the Statement of
Penalties/Receivables which we received from your good office through e-mail on 10
March 2022.

In the said Statement, the 5% down payment amounting to PHP 202, 671.48 was
scheduled to be settled by 31 March 2022. However, we were unable to pay the same. In
this regard, we humbly request for an extension to pay the 5% down payment until 15
April 2022.

We hope that you consider our foregoing requests.
