Subject: Request for Permission to Conduct a Survey

We, the undersigned, are students at Lanao del Norte Provincial Science and Technology High School
conducting a survey as part of our practical research. The purpose of this survey is to gather information
on the effects of chronic absenteeism to academic performance.

We humbly request your permission to conduct this survey among the Senior high school students of
Lanao del Norte Provincial Science and Technology High School. The survey will be conducted during this
month of February and will be administered in a manner that minimizes disruption to the normal school
activities.

We assure you that all responses will be kept confidential and will only be used for the purpose of our
project. The results of the survey will be used for academic purposes only and will not be published or
distributed publicly.

We understand the importance of obtaining consent for such activities and assure you that we will
adhere to all guidelines and regulations set forth by the school.
