In response to your Notice of Call for Accreditation, kindly be informed that Calamianes
Seaweeds Marketing Cooperative (CSMC), with office address Sitio Balisungan, Barangay Tagumpay,
Coron, Palawan would like to seek accreditation to the Municipality.

In support of this application are the following administrative requirements:

1. Duly accomplished Application Form for Accreditation;

2. Duly approved Board Resolution signifying intention for accreditation for the purpose of
representation in the local special body;
3. Certificate of Registration issued by the CDA
4. Mayor’s Permit
5. BIR Permit
6. List of Current Officers;
7. CY 2023 Minutes of the Annual Meeting;
8. CY 2023 Annual Accomplishment Report/ CAPR; and
9. CY 2023 Financial Statements
