Dear Sir/Madam:

Greetings of peace and goodwill!

We are writing on behalf of the Scouting Units of Putlod-San Jose National High
School: the Boy Scouts of the Philippines and Girl Scouts of the Philippines. We humbly
request your consent and approval to conduct the 2023 SCHOOL ENCAMPMENT at
Putlod-San Jose National High School for our Boy and Girl Scouts from Grades 7 to 12,
scheduled for December 2-3, 2023, Saturday and Sunday, respectively, at Putlod-San Jose
National High School, Putlod, Jaen, Nueva Ecija.

Our proposed theme for this momentous event is "Sustainable Development

Goals: Engaging the Scouts toward a Sustainable Future." We firmly believe that this
theme is in alignment with our present global challenges and resonates deeply with the
values we want to instill in our young scouts. Our principal objectives for the camp include:
1. Awareness & Education: To provide an avenue for the scouts to be educated
about the importance of sustainable development goals and their role in
achieving them;
2. Skills Enhancement: To reinforce essential scouting skills and impart new
learnings that will be beneficial for their personal growth;
3. Teamwork & Unity: To strengthen camaraderie, teamwork, and unity among
the scouts through various team-building exercises; and,
4. Character Building: To further shape their character and values, ensuring they
grow as responsible and environmentally conscious individuals.

The set of activities we have organized to meet these objectives includes:

1. Scouting Skills Training: Workshops to bolster their scouting skills and
knowledge;
2. Team Building: Activities that emphasize the importance of unity, trust, and
collaboration;
3. Peace Jam: An interactive session to promote peace, understanding, and
harmony among participants;
4. Forum: An open discussion on sustainable development goals, their importance,
and the role of each scout in achieving them; and,
5. Campfire: A traditional scouting event to end the day, fostering brotherhood and
sharing of experiences.

CONTINGENCY PLAN
2023 School Encampment

1. Objective of the Encampment

2. Pre-Encampment Preparations
 Risk Assessment: Identify potential hazards (e.g., weather conditions,
wildlife, health emergencies).
 Safety Briefings: Conduct briefings for staff and students on emergency
procedures.
 Equipment Check: Ensure all necessary equipment (tents, first aid kits,
communication devices) is in good condition.
 Training: Provide training for staff and senior scouts on first aid, emergency
response, and leadership skills.

3. During the Encampment

 Periodic Briefings: Hold morning and evening briefings to discuss the day's
activities and any safety concerns.
 Activity Monitoring: Supervise all activities closely, with a focus on safety.
 Emergency Protocols: Establish clear procedures for various emergencies
(injury, lost participant, severe weather).
 Health and Hygiene: Monitor health conditions and promote good hygiene
practices to prevent illness.

4. Emergency Response Plans

 Injury or Illness: First aid response, transportation to medical facilities, and
parental notification.
 Lost Participant: Search and rescue protocols, including roles for staff and
communication strategies.
 Severe Weather: Procedures for sheltering or evacuating, depending on the
severity.
 Fire: Fire safety measures, evacuation routes, and assembly points.

5. Communication Plan
 Internal Communication: Radios or phones for staff members for quick
coordination.
 External Communication: Establish contact with local emergency services
and parents.
 Information Sharing: Regular updates to parents and school
administration.

na tagapangalaga ng Scout na si ____________________________________________,
isang rehistradong mag-aaral ng Putlod-San Jose National High School, mula sa
___________________ (Grade and Section), ay nagpapahintulot na lumahok ang aking
anak sa 2023 SCHOOL ENCAMPMENT na gaganapin sa December 2-3, 2023 sa
Putlod-San Jose National High School, Brgy. Putlod, Jaen, Nueva Ecija.

Nauunawaan ko na ang paglahok sa mga aktibidad ng Scouting ay may

kaakibat na panganib at hirap sa aspetong pisikal, mental, at emosyonal. Maingat
kong pinag-isipan ang mga panganib na kaakibat nito at nagbibigay ako ng
pahintulot para sa aking anak na lumahok sa nasabing aktibidad. Nauunawaan ko
rin na ang paglahok sa aktibidad na ito ay LUBOS NA BOLUNTARYO at
nangangailangan sa mga lumahok na sumunod sa mga naaangkop na patakaran,
regulasyon at mga pamantayang pangkaasalan. Pinapawalang-sala ko ang Boy
Scouts of the Philippines at Girl Scouts of the Philippines, ang mga Local Councils,
mga koordinator ng aktibidad, at lahat ng propesyonal na tauhan, mga boluntaryo,
kaugnay na grupo, o iba pang organisasyon na kaugnay sa aktibidad mula sa lahat
ng claims o pananagutan na magmumula sa paglahok na ito.

Sa kaso ng ano mang emergency kasangkot ang aking anak, nauunawaan

ko na gagawin ang lahat ng pagsisikap upang ma-contact ako. Sa pangyayaring
hindi ako maaabot ng komunikasyon, binibigyan ko ng pahintulot ang medical
provider na pinili ng adult leader na nangangasiwa na makakuha ng nararapat na
paggamot, kabilang ang hospitalisasyon, anesthesia, operasyon, o iniksyon ng
gamot para sa aking anak. Pinapahintulutan ang mga medical providers na ihayag
sa adult leader ang mga natuklasan sa examination findings, test results, and
treatment para sa layunin ng medical evaluation sa lumahok, susunod na pag-
follow up at komunikasyon sa mga magulang o legal na tagapangalaga ng
lumahok, at/o pagtukoy sa kakayahan ng lumahok na ipagpatuloy ang mga
aktibidad sa programa.
