We hope this letter finds you well. We are the upcoming nursing graduates in College of Our Lady of Mt Carmel
(COLMC), and we are writing to formally request permission to borrow the O.R. logbook at Our Lady of Mt Carmel
Medical Center (OLMCMC). The purpose of this request is to facilitate the completion of PRC (Professional Regulation
Commission) cases for the preparation of taking the PNLE exam this May 2024.

As part of our academic and professional development, we are required to document and submit our practical
experiences. The completion of these cases is crucial for our eligibility and eventual licensure with the PRC.

To fulfill these requirements, we kindly seek your approval to borrow the O.R. logbook on December 13, 2023 needed
to record the necessary information. The logbook will be handled with the utmost care and returned promptly upon
completion of the documentation.

We understand the importance of maintaining the confidentiality and integrity of the records within the logbook. Rest
