at ___________________, hereby name, constitute and appoint MARIA JUANA
DELOS SANTOS, of legal age, Filipino, married and with residence address at
________________________ to be my true and lawful attorney-in-fact, for me
and in my name, place and stead, to do and perform the following acts and

1. To transact with the Land Transportation Office (LTO) in relation to

the lost original Official Receipt/Certificate of Registration (OR/CR) of

in requesting/applying for a certified true copy or re-issued copy of the

from BDO Makati in 2012;

4. To make, sign, execute and deliver any and all documents necessary
in requesting for a certified true copy of the said registered chattel
mortgage or any other documents in relation thereto;

5. To receive any and all documents pertaining to the above transactions.

HEREBY GIVING AND GRANTING unto my said attorney-in-fact full

power and authority to do and perform any and every act and thing whatsoever
requisite, necessary or proper to be done in and about the premises as fully to
all intents and purposes as I might or could do if personally present and acting
in person; and

HEREBY RATIFYING AND CONFIRMING all that my said attorney-in-

fact shall lawfully do and cause to be done under and by virtue of these
