An oxygen flash fire occurred during the assembly of an oxygen cylinder rack at AIGCO on June 7, 2018. While opening an oxygen cylinder valve, a flash fire occurred at the pressure gauge. The root cause was determined to be traces of oil found in the pressure gauge. Requirements for cleaning equipment for oxygen service were not specified in the purchase order for the new manifold. There were also gaps in following procedures for changing cylinders from one gas service to another.
