This is to remind you that in order for us to process your final pay and/or back pay, the following are your
your obligations upon termination of your probationary contract:

• Finish All Paper works

• Pay any liabilities
• Surrender Company Property
• Forward mall id
• Forward all other identification bearing the company’s name, all manuals, records, trainee
materials
• Other confidential information, equipment, an itemized listing of all services completed
• Turn over of Inventory of all items

Kindly coordinate with your immediate superior for the processing of your clearance. As of date, we have
not received your clearance from the branch. Be guided that once your clearance has been transmitted
from branch to office, we shall be processing the said document within 30 days upon receipt.
