SUBJECT: CONDUCT OF PHYSICAL FITNESS TEST (PFT) CY 2022

DATE: 16 September 2022/ Friday

1. References:
a. PPMP CY 2022
b. Memorandum Circular No. 2019-017 re: Amendment Policy
BFP Health and Fitness Program signed by the Chief BFP dated
05 August 2019.
c. Memorandum dated 15 September 2022 re: Above Subject
signed by CSUPT ROBERTO M GENAVE, DSC, BFP Regional
Director IV-A.

2. In view of the above references, be informed that the conduct of

Annual Physical Fitness Test (PFT) is scheduled on 27-29 September 2022.

3. In line with this, direct all BFP officers and personnel assigned within Area
of Responsibility (AOR) to undergo the following laboratory examination at any
government diagnostic laboratory/hospital, to wit;

a. ECG
b. Chest X-ray
c. CBC
d. Urinalysis
e. Blood Chemistry (FBS, Lipid Profile, Creatinine & BUA)
f. Pregnancy Test Serum (female)

4. However, BFP personnel who have personal health card may undergo
the above-mentioned laboratory examinations in any affiliated DOH accredited
diagnostic/laboratory.

5. Further, Station Chief EMS shall consolidate the aforementioned

laboratory results using the attached format and submit NLT 22 1200H September
2022 in excel and pdf format thru email address: bfpquezonems@gmail.com
