On application of the above named Creditor for an Order to Pay Garnishment Proceeds to the Creditor, the Court finds and
accordingly ORDERS:
1. An Order for Non-earnings Garnishment was served on the Garnishee on ____________________________________.
2. Garnishee responded by Answer sent to the Creditor and Debtor on ____________________________________.
3. The Court finds that the Garnishee holds the sum of $________________ subject to the Order of this Court from the
accounts or amount due Debtor.
4. The Creditor verifies that immediately after the Order of Garnishment was served on the Garnishee, the Creditor served
a Notice to Debtor of Exemptions Non-earnings Garnishment on the Debtor as required by law.

5. More than 14 days have passed since the Notice of Exemption was served on the Debtor and no exemption hearing has
been requested or an exemption hearing was requested and held, and the Court determined the amount set forth herein
was not subject to exemption, and no reply has been made to the Answer of the Garnishee within 14 days following the
date the Garnishee completed the Answer.
6. The Garnishee is hereby ordered to pay to the Creditor if unrepresented or the Creditor’s attorney whose name and
address is shown above, the amount shown in paragraph 3 above less the administrative fee, if claimed, or such lesser
sum if a lesser sum is ordered paid to the Creditor by the Court, in which case the balance shall be released to the
Debtor. ** Show case name and number on remittance.
7. The Creditor shall promptly refund to the Debtor any overpayment of the claim.
8. Upon compliance by the Garnishee with paragraph 6 hereof, the Garnishee shall be released from all further liability
arising under this Order of Garnishment.

* This Order not to be entered until 14 days after service of Notice of Exemptions on the Debtor and if the Debtor has filed no request for
exemption hearing, and at least 14 days after Garnishment Answer was sent, and if neither the Debtor nor the Creditor has filed a reply to
the Garnishment Answer.
** Banks, savings and loan associations, credit unions, and finance companies may, but are not required, to claim $15 as an administrative
fee. Such fee is in addition to the amount required to be withheld, except that if the amount required to be withheld exceeds the funds
available, the fee is deducted from the amount withheld.
