AFFIDAVIT

I, ARIANE MAE NOEL, Filipino, single, of legal age, with resident

address at Blk 5 Lot 27 PH 1 NHV Bitungol Norzagaray Bulacan,
after being sworn according to law depose and state that:

1. I am a holder of Philippine Passport No. P8534829B, issued at

DFA Manila;

2. The validity of my passport is up to 20 December 2031; a copy

of which is hereto attached as ANNEX “A”;

3. However, my said passport is “tainted” when my lipstain got in

contact with it, leaving smudges on the first three (3) pages

4. I executed this affidavit to aver the truth of the foregoing and

for purposes of securing a new passport and for whatever legal
purpose it may serve.

IN WITNESS WHEREOF I have hereto affixed my signature this 16th day

of February 2024 in Las Piñas City.
