TRAINEE- INDUSTRY AGREEMENT LIABILITY WAIVER

To Whom It May Concern:

I, ANGELINA MAE G. MOSO, student of Cavite State University – Bacoor City Campus,
has been granted permission to undergo an on-site on-the-job training at LOTUS CENTRAL
MALL INC. with principal address at 4th floor Tanzang Luma nueno avenue Imus, Cavite
starting March___, 2024.

Further, I hereby voluntarily agree to undergo the said training in the company/firm in
order to acquire actual work experiences with the following terms and conditions:

1. That I, understand that the company/office will not be liable for any untoward
incident that may happen while on my way to the training site and in performance of
my duties and functions as a trainee;
2. That I, shall abide with the company/office rules and regulations and shall comply
with the imposed requirements of the on-site on-the-job training;
3. That I, shall exercise care and diligence in any task assigned to me; and
4. That I, shall be made answerable in all liabilities for damages to property caused by
my negligent acts while on the course of training.
