Intended recipients
This communication has been sent to all County Directors of Human/Social Services,
Collaborative Management Program Coordinators, and Collaborative Management Program
Stakeholders. Please forward pertinent information to staff members as you deem necessary.

Purpose
The purpose of this memorandum from the Office of Children, Youth, and Families is to
outline data standards and expectations for the Collaborative Management Program (CMP) and
to provide guidance regarding data collection.

Action
Collaborative Management Program (CMP) sites are to follow the standards described in this
memo. CMP sites must enter all participants of the CMP into either the Comprehensive Child
Welfare Information System (CCWIS)/Trails or Efforts to Outcomes (ETO) database. ETO shall
be used for non-child welfare children, youth, and families to track participation. Trails
database shall be used for all Child Welfare CMP-served children, youth, and families.

The CMP site is responsible for ensuring there is no duplication of clients entered into ETO
and/or Trails. Duplication is defined as a child, youth, or family that is counted twice for the
same ISST meeting or prevention program and recorded in one (1) or more CMP data
system(s). A child, youth, or family may be counted for multiple service episodes supported
by several multi-system partnerships.

For those participants entered into ETO, first name, last name, date of birth, gender,
race/ethnicity, zip code, disability status, victim status, and referral source must be entered
into the database. CMP sites are also required to collect and report data on CMP ISST clients’
recommended services and a description of outcomes for children served as well as including
a description of the services that were recommended but not provided and a description of
the barriers to providing such services. CDHS is working on a data collection solution for
participants entered into Trails.

Background
In 2014 the Colorado Department of Human Services agreed to improve data collection and
reporting protocols for the programmatic and expenditure data and require all county
stakeholders that participate in county-level CMP to comply with those standards. In May of
2023, HB23-1249 was passed, changing the original CMP statute (CRS 24-1.9-101-104). As a
result, there are new data collection requirements.
