SECRETARY’S CERTIFICATE

I, ANGELICA P. SAMAR, of legal age, Filipino, single, and with office address at Barangay
Codiong, Libertad, Antique after having been sworn to in accordance with law, do hereby depose and
say:
1. That I am duly appointed Sangguniang Kabataan Secretary of Barangay Codiong, duly
organized and existing under and by virtue of the laws of the Philippines with office and
place of business at the above-stated address;
2. That as such, I am the custodian of the books and records of said office, including the
Minutes of Meetings and Resolutions of its council;
3. That on January 22, 2024, the council unanimously approved and adopted the following
resolution: RESOLUTION N0. 06 Series of 2024.
“HEREBY RESOLVED, to DESIGNATE AND AUTHORIZE HON. LOVELY ROSE A.
MERCADO AND SK TREASURER MS. JENNYVIVE E. ORCANA AS AUTHORIZED
SIGNATORIES OF BARANGAY CODIONG, LIBERTAD, ANTIQUE WITH AN
ACCOUNT NO. 003372-1053-98 AT LANDBANK OF THE PHILIPPINES, CULASI,
BRANCH, CULASI, ANTIQUE.”
RESOLVED FINALLY, that a copy of this resolution be furnished to officer and/or manager
of Landbank of the Philippines, Culasi, Antique Branch.
4. That the foregoing Resolution has not been revoked, superseded, cancelled, modified or
amended and the same is still in full force and effect as of the date of this Certificate.
