8.1.1. The meeting started with an Opening Prayer lead by Mr. Eugenio Carinan.

8.2.1. Mr. Hidalgo discussed the bases of establishing of CQIP. To start, Mr. Hidalgo present a
sample of CQIP from USI main campus which includes projects and program for
Educational aspect.
8.2.2. Base from the summary of Domains given by the QAD, GSSD shall be under Domain 4:
Healthcare Environment. For Healthcare Environment, the division shall eliminate the
following: Old and broken Signage, delay in service from Maintenance, unfollowed
procedures in Work Order System by some Hospital members, customer complaints
regarding room repair and obsolete room items and attendance of employees for
regular monthly meetings.
