8. MINUTES:
8.1. Opening Prayer
8.1.1. Mr. Daniel Baracena, TQM conducts a re-orientation to the Division Director, Section
Supervisors and Heads regarding OTP.
8.1.2. The orientation/meeting started with an Opening Prayer lead by Mr. Baracena.
8.1.3. After the re-orientation, the formal Division meeting was started. A brief review of
previous meeting was conducted and then asked if there are corrections in the
previous meeting.
8.1.4. Mr. Carinan moved to approve the previous minutes with minor correction and duly
seconded by the body as corrected.
8.2. Business Arising from the Previous Minutes
8.2.1. Borrowing of DRMC Cart is still pending.
8.2.2. Ms. Briones said that the Housekeeping Section have already received a complete set
of uniform. For Maintenance Section, only (1) one set of uniform was released to a

few Maintenance Staff. Ms. Amy said to monitor the compliance of wearing the
prescribe uniform.
8.2.3. Room 241 for retouching only.
8.2.4. Request for double bucket is still pending.
8.2.5. Mr. Velasco suggests if possible to replace medium size oxygen tank to smaller one.
8.2.6. The Linen Section request for silk screen printing for the coding of linen.
8.2.7. To follow-up the request of additional Orderly for Dialysis Section.
8.2.8. Engr. Efondo present a sample design of BP App. Ms. Amy said to look for other design
for more options. Asks MMS for some suggestions.

8.3. New Business


8.3.1. Reminders
8.3.1.1. Bomb Awareness. The Maintenance Section is requested to attend the Bomb
Awareness on October 31, 2018 2PM at Seton Hall.
8.3.1.2. Culminating Activity for the Month of Holy Rosary. Ms. Joana discusses the
flow of the activity.
8.3.1.3. To request for Barricade and signage at Gate 3. The Gate will be used for EXIT
ONLY and only be opened for Deliveries and Emergency.
8.3.1.4. Request for Industrial Fan – on going.
8.3.1.5. HRD Memo no. 26 s. 2018 –Deployment of New Security Officers was
downloaded.
8.3.1.6. QAD Memo No. 19 s. 2018 – Fire Drill was downloaded. PM of Fire
Extinguisher and Emergency Light must be updated said Ms. Amy. A Fire Drill
Scenario will also be conducted.

8.3.2. Report of Every Area


8.3.2.1. Maintenance Section
8.3.2.1.1. Siphoning of Septic Tanks near Magnificat Homes and Sister
Quarters was conducted last October 20-21, 2018.
8.3.2.1.2. Water Supply at Rooms 406-409 Salvation Unit was cut due
clogged. The Maintenance Section resolves the incident by
manual repair and cancelled the WOS request materials.
8.3.2.1.3. The “NO ENTRY” and “ONE WAY” signage at Gate 3.
8.3.2.1.4. Fabrication of Water Tank Cover at Magnificat Home was already
done.
8.3.2.1.5. On-going fabrication of Lighting Bracket of “UNIVERSIDAD DE
STA. ISABEL HEALTH SERVICES DEPARTMENT” signage at the back
of Doctors Clinic.
8.3.2.1.6. Monitoring of STP is conducted daily with logbooks.
8.3.2.1.7. Request of Clamp Ammeter has already been released. Mr.
Carinan said that the MMS bought local brand of Clamp
Ammeter.
8.3.2.1.8. The 10 days Orientation of new applicant for Welder has already
been started.
8.3.2.1.9. Installation of Chlorinator at Concrete Water Tank, Annex and
STP has already been done.

8.3.2.1.10. On-going projects:


8.3.2.1.10.1. Installation of Exhaust fan at Room 112 and 114.
8.3.2.1.10.2. Roofing of Generator House of 500 and 300 KVA.
The request materials have already been available.
8.3.2.1.10.3. Closing of open ventilation at Generator House.
Sister Lolit and Sister Geraldine with Engr. Evora
and Engr. Dadua suggested to request for
Aluminium door for 300 and 500 KVA gen. set
house.
8.3.2.1.10.4. Electrical Manhole cover at STP.
8.3.2.1.10.5. Fabrication of Boom at Gate 3
8.3.2.1.10.6. For repair of Washing Machine at Landry area.
8.3.2.1.10.7. For repair of Ceiling at X-ray.
8.3.2.1.10.8. Installation of Motor compressor and fan blade at
OR 4.
8.3.2.1.10.9. Establish POP on Sewage Treatment Plant.
8.3.2.2. Linen/Laundry/Sewing Section
8.3.2.2.1. For repair of Old Washing Machine.
8.3.2.2.2. For repair of Squeezer.
8.3.2.2.3. To follow-up the request ceiling for repair.

8.3.2.3. Biomedical Section


8.3.2.3.1. Defective parts of X-ray Digitalizer were already surrendered to
MMS for back-up and replaced with new one.
8.3.2.3.2. South Side is not compliance with the requirements needed like
Service report.
8.3.2.3.3. PM of Autoclave machine at OR is now under RCJL.
8.3.2.3.4. PM at Clinical Lab and Blood Bank has already been done.

9. Other matters:
9.1. Mr. Elmer request if possible the Maintenance Staff should report to the GSSD (Mr. Elmer) for
the status of projects. (WOS request especially major projects. Sister Lolit is asked for an update
time to time)
