VPN Access Request

All fields are required. Incomplete Form will not be accepted. No exceptions.

The Bank reserves the right to terminate VPN connection or user account if a violation of the Remote Access policy is observed or if
the Bank perceives that the connection represents a threat to the Bank’s network and information systems. Users are prohibited from
sharing their security credential such as the User ID and password. The user must comply with the Bank’s Information Security
Policy and Procedures with regards to password security standards.

Standard of Conduct:
I understand that:
- VPN access is a privilege and may carry risk for EWB; therefore my access may be revoked at any time.
- All hardware and software are property of EWB. I shall return all hardware and software assigned and in my possession upon my termination of employment with
EWB.
- I will not distribute or share my user name or password with anyone.
- I will not store EWB information on my home computer or removable media nor will I share EWB information with anyone outside of the Bank, including
customer’s confidential information and the Bank’s proprietary information.
- I understand violation of these terms will subject to my VPN access being revoked and action taken up to termination.
- If I lose my VPN token (if one was issued) I must contact the Help Desk at (626) 768-6011 immediately.

* In order to remotely access the Bank’s private network via VPN, your PC must be in compliance with below security control requirements:
- Your PC must have the latest anti-virus software, with virus signature file kept up to date providing real-time protection.
- Your PC must have a firewall turned on to protect the PC from any harm such as malware/virus while connecting to the Bank’s private network.
- Your PC must have the latest patches installed to protect the PC from any harm such as malware/virus while connecting to the Bank’s private network.
- While your PC is connected to the Bank’s private network, the same rules/policy applies for Internet Access, and therefore, every precaution and due diligence
must be taken to protect the Bank’s network and information systems from any harm.
- It is imperative to ensure that no customers’ confidential information and the Bank’s proprietary information are being saved on your personal PC.
