SUBSCRIPTION FORM (AY 2023-2024)

TERMS AND GUIDELINES

1. Subscription to the Yearbook 2023-2024 is MANDATORY.

2. Graduating siblings from different colleges/departments may subscribe to only one (1) yearbook, provided they
notify the Yearbook Adviser in writing prior to completing the subscription form.
3. Graduating students must submit their duly filled out subscription forms during the scheduled pictorial.
4. The subscription form, duly filled out by the graduating student and stamped as received by The
Augustinian Yearbook Office, serves as a contract between the two parties, with no allowance for cancellation
or withdrawal of the subscription form.
5. The graduating student shall pay the subscription fee amounting to P2,500.00 directly to the Accounting Office.
6. The Augustinian Yearbook Office reserves the right to forfeit the subscriber’s space in case they cannot comply
with the duly communicated requirements arising from the pre-production phase of the yearbook.
7. The subscriber may correct the entry but must notify the Yearbook Adviser in writing within 30 days after the
pictorial.
8. The release of the yearbook is one (1) year after the printing and production of Yearbook 2024.
9. The University retains the right to dispose the yearbook if not claimed after three (3) years of its release.

In compliance with the Data Privacy Act (DPA) of 2012, and its Implementing Rules and Regulations (IRR) effective
since September 8, 2016.

I agree and authorize the USA Yearbook Office to:

1. Retain my information for a period of one year from the date of termination of my policy or at such time that I
submit a written cancellation of this consent to the USA Yearbook Office, whichever occurs earlier. I agree
that my information will be deleted/destroyed after this period.

2. Collect and share my information with affiliates and necessary third parties for any legitimate purpose.

I also acknowledge and warrant that I have acquired the consent from all relevant parties pertaining to this
consent. I hold the USA Yearbook Office free and harmless and indemnify them from any complaints, suits, or
damages that any party may file or claim in relation to my consent.
