AND CONSENT TO USE THE SURNAME OF FATHER

I, RODELITO L. RANOCO, of legal age, Filipino, married,

and a resident of Brgy. 3, San Francisco, Agusan del Sur,
after having been duly sworn to in accordance with law, do
hereby depose and say:

1. That I am the biological father of MIGUEL GABRIEL

SIDENA GIMENA ;

2. That he was born on August 9, 2000 at Davao Medical

Center, Davao City ;

3. That I was not legally married to his biological

mother ;

4. That I hereby acknowledge my paternity/filiation of

the child, MIGUEL GABRIEL SIDENA GIMENA; and I am
giving my consent for the child to use my surname and
that the same be reflected in his birth
certificate; ;

5. That I am executing this affidavit to attest to the

truthfulness of the foregoing statements and for
whatever purposes, legal and legitimate.
