AFFIDAVIT OF WITNESS (Owner)

I, Jonathan A. Santos, of legal age, married, and residing at Poblacion, Kalibo Aklan, after
having been sworn to in accordance with law, hereby depose and state:
1. I am the owner of Fresh Mart, a grocery store located at XIX Martyr’s St. Kalibo, Aklan,
as identified in the foregoing narrative.
2. On the 8th day of February, 2024, I was informed by Mr. Miguel Pelaez, the store
manager, of a suspicious incident involving our trusted cashier, Sarah M. Rodriguez.
3. Mr. Pelaez showed me the CCTV footage which clearly captured Sarah M. Rodriguez
taking cash and company property from the manager's office without authorization.
4. Based on the evidence presented to me, it is clear that Sarah M. Rodriguez has
committed theft against Fresh Mart by unlawfully taking cash amounting to ₱81,200.00,
a company laptop valued at ₱30,999.00, and a company cellphone valued at
₱36,490.00.
5. I hereby affirm the truthfulness of the statements made herein and I am willing to testify
to the same in any legal proceedings arising from this incident.

I executed this affidavit to attest the truthfulness of the foregoing facts and to support the
filing of Criminal Cases against ____Sarah M. Rodriguez____ for violations of
_________Qualified Theft__________.
