AFFIDAVIT OF LOSS

I, KARL STEVENT S. CABARLES, of legal age, Filipino, single and resident of 019
Corinthian St. Dona Maria Subdivision, Barangay, Tenejero, Balanga City, Bataan, 2100,
after being sworn according to law, hereby depose and state that:

1. I am the owner of the Duplicate Transfer Certificate of Title of a parcel of land (TCT
No.031898) issued by the Registrar of Deeds on March 18, 1998 at LRA Building,
Provincial Capitol Compound, Banzon St. Balanga, Bataan, 2100.

2. Sometime in December 20, 2023, I lost the above Duplicate Transfer Certificate of Title
under the following circumstances:

That on or about 8:00 in the evening of the above-mentioned date, a fire engulfed a portion of
my house due to the overheating of the wires of the christmas lights I installed. The portion of
the house engulfed by the fire was my bedroom and inside the said room, there was a wooden-
vault where the Duplicate Transfer Certificate of Title was found. The fire spread across the
room which burned majority of my personal belongings, including the said wooden vault.

3. Despite diligent efforts exerted, I could not longer recover the said Duplicate Transfer
Certificate of Title, and to the best of my knowledge, the same is truly lost.

4. The said Duplicate Transfer Certificate of Title has not been confiscated by any officer of
the law or the issuing office or agency due to the violation of any law, statute, order, rule,
or regulation.
 5. In the event that the lost Duplicate Transfer Certificate of Title should hereafter be found
or located, I undertake to forward or surrender the same to the Registrar of Deeds for
appropriate disposal.

6. I am executing this Affidavit to the truth of the foregoing statements and to request the
issuance of a new Duplicate Transfer Certificate of Title in lieu of one which was lost.
