I, ROWENA DELA CRUZ BAUTISTA, of legal age, Filipino, single, resident of No. 295
Purok 3 Barangay Sta. Maria Camiling, Tarlac, after having been sworn to an oath in accordance with
law, do hereby depose and state:

That, I am the person named in the birth certificate issued by the Philippine Statistics
Authority (PSA);

That, I am the person named in the identification card issued by the Professional Regulation
Commission (PRC) as a registered CPA from May 20, 2009, renewable every three (3) years subject
to the rules of the profession;

That said credentials or documents are correct, valid, and authentic;

That, I am executing this affidavit to attest to the truth of the foregoing circumstances and to
attest to the validity and authenticity of the documents submitted;
