I, James authorize monthly automatic payments of $40.00 beginning on 1st day of the
month. Also the annual maintenance fee of $59.00 due on 9/15/2022.

If I choose to cancel my monthly membership, I need to give a 30 day written notice

from my last billing date (in person or mailed letter). Otherwise, my membership will
automatically deduct monthly payments on the 1st day of the month. I also understand
that if I miss a payment, I forfeit my membership agreement which will result in a late
fee as well as a suspension of my membership. Ever Fitness is a private club,
independently owned and operated, and reserves the right to modify or revoke my
membership at any time.

By affixing my signature to the document, I attest contract understand & agree that I’m
legally bound by its contents. In consideration for my allowance to use the Ever Fitness
facilities on a membership basis, I hereby stipulate & agree;

That I’m fully cognizant & mindful of the serious consequences which will result due to
my involvement of weight lifting, strength training, and body building while at Ever
Fitness.
That I’m in good health & have no physical limitations that would preclude my
involvement .

I’m forewarned that Ever Fitness will not in any event provide medical and/or
hospitalization insurances for my benefit in the event of any injury to my person as a
result of my being on any portion of the premises.

I acknowledge & specifically intend that this release & waiver shall be effective not only
on the above date hereof but also on all occasions in the future when I shall use Ever
Fitness facilities on a membership basis.

Having read the above terms, I also understand this document to be complete waiver
and disclaimer of Ever Fitness.
