CONTRACT OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Contract of Agreement is entered into and executed by between EDWIN JURILLA/MYLAH S.
DEVEZA of legal age, Filipino, and a resident of TAMBOBO DUMANGAS, ILOILO, Philippines, hereafter
called FIRST PARTY; and ROSALIE BONGHANOY; of legal age, Filipino, single and resident of CAPALIZ
DUMANGAS, ILOILO Philippines, are all hereinafter called, SECOND PARTY.

WITNESSETH:

WHEREAS, the First Party is owner/operator of one (1) unit motor vehicle with CPC under LTFRB
Case No: 01-741-AK (PUV) described as follows:

WHEREAS, the Second Party is acting for and behalf of her Family:

WHEREAS, the unit of the First Party was hired by the Second Party based in Dumangas, Iloilo to
transport said passengers (FAMILY) effective June 3-7, 2023 from place of Origin: DUMANGAS, ILOILO to
place of destination: ILOILO INTERNATIONAL AIRPORT, CABATUAN, ILOILO AND VICE VERSA;

WHEREAS, the First Party signify his Conformity and agree to the desire of the Second Party to
use his unit for the said purpose subject to existing laws, rules and regulations of the LTFRB and to
follow protocols imposed by DOH regarding COVID-19;
NOW THEREFORE, for and in consideration of the foregoing premises, the parties hereby bind
themselves to comply with above terms and conditions of this agreement.

request for Special Permit to allow service passengers from Dumangas, Iloilo to Iloilo
