List of Requirements Present in the Second Set but

Absent in the First Set:


1. Certification from the Housing and Land Use Regulatory Board (HLURB)
Regional Officer on the actual zoning or classification of the land (LUC
Form No. 5).
2. Certification from the authorized Department of Agriculture (DA)
official stating various details about the land (LUC Form No. 6).
3. Certification from the authorized DENR official stating if the land falls under
specific protected areas or environmentally critical areas (ECA) (LUC Form
No. 6).
4. Environmental Compliance Certificate (ECC), if applicable (when the land is
within an ECA or involves an ECP).
5. DAR Clearance issued by the DAR Provincial Office (No. 30).
6. Tax Payment Certification from the City Assessor of the land (No. 31).
7. BIR Certification as to the Zonal Value of the land intended for conversion
(No. 32).
8. Verification/Certification of Truthfulness & Correctness (pursuant to AO 1
Series of 2019) (No. 32).

Note:

 The requirement for HLURB Certification (No. 15) in the first set is replaced
by Certification from the HLURB Regional Officer (No. 1) in the second set.
 In the second set, instead of choosing between SB/SP Zoning
Ordinance and SP Resolution Approving Ordinance depending on the
location (No. 16), both are required.
