CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This contract made and entered by and between:

Municipal Government of Nasugbu with office address at the Municipal Hall,

Escalera St. Nasugbu , Batangas, herein represented by Mayor Antonio A. Barcelon
Municipal Mayor, hereinafter referred to as the “First Party”.

Dalmas A. Binumnga Jr. of legal age, Filipino and with residential address at
Valenzuela St. Cor. V..Mapa Sta Mesa herein after referred to as “Second Party.

-WITNESSETH-

1. That the First Party is in need of the services of the Second Party who shall
perform work that will temporarily be needed by the First Party;

2. That the Second Party has signified his/her intention to which the First Party has
accepted, to provide the service needed by the latter.

3. That the Second Party hereby possesses the education, experience and skills
required to perform the job as described herein;

4. That the Second Party hereby attest that he/she is not related within the fourth
degree of consanguinity or affinity to the; 1) hiring authority and/or 2) representative
of the First Party; that he/she has not been previously dismissed from the
government service by reason of administrative offense; that he/she has not already
reached the compulsory retirement age of sixty-five (65).

5. That in view hereof, the Second Party is hereby contracted as Medical Officer for
the period of January 1,2023 to December 31, 2023 in consideration of a daily rate
of EIGHTTHOUSAND PESOS ( P8, 000.00 ) for a 24-hour actual service rendered
per day with a maximum of EIGHT (8) days actual service per month or equivalent to
Sixty Four thousand pesos ( P64,000.00 ) per month.

6. That payment will be made by the First Party within seven (7) working days after
the full agreed required services for the month has been rendered by the Second
Party and properly recorded and certified by the Chief of Hospital of Ospital ng
Nasugbu (OSNA).
7. That as Medical Officer, the Second Party is expected to perform the following
functions:

6.1. Assist the Chief of Hospital I, Ospital Ng Nasugbu (OSNA) in the

efficient and effective delivery of medical services to the constituents of the
Municipality of Nasugbu;
6.2 Conduct medical consultation and treatment within the municipality.
6.3. Perform other related duties as may be required from time to time by the
Chief of Hospital, ON and/or the Municipal Mayor.

8. That the Second Party shall perform work at a time and schedule to be agreed
upon by both parties;

9. That it is understood that this contract does not create an employer-employee

relationship between the First Party and the Second Party, that the services
rendered hereunder are not considered and will not be accredited as government
service, and that the latter is not entitled to benefits enjoyed by regular personnel of
the First Party.

IN WITNESS WHEREOF, both parties have hereunto set their hands this
day of 2023 at Nasugbu,
