AFFIDAVIT OF LEGITIMATION

We, JOHN MICHAEL MONTERON MAHUSAY and APRIL DINOY YBAÑEZ,

both of legal age, husband and wife, respectively, and presently residing at 926 San
Roque St. Brgy. Hipodromo, Cebu City, Philippines after having been duly sworn to in
accordance with law, depose and state:

1. That we are the biological parents of JUAN MIGUEL YBAÑEZ MAHUSAY,

who was born on 12 March 2020 at University of Cebu Medical Center,
Mandaue City;

2. That his birth was recorded at the Local Civil Registrar of Mandaue City on 27
March 2020 under Registry No. 2020-02842;

3. That at the time of his birth, we, the biological parents were not yet married;

4. That at the time when he was conceived, there was no legal impediment to
contract marriage between us, his biological parents;

5. That we subsequently contracted marriage on 16 June 2020 at Municipal Trial

Court, Minglanilla Cebu;

6. That our marriage was solemnized by Judge Wilson T. Ibones and was
recorded at the City Civil Registrar of Minglanilla, Cebu on June 18, 2020
under Registration No. 20200455;

7. That by virtue of our subsequent marriage, the said child JUAN MIGUEL
YBAÑEZ MAHUSAY, is now legitimated by operation of law; and

8. That we are executing this Affidavit to attest to the foregoing facts, for
purposes of complying with the requirements in applying for the legitimation
by subsequent marriage of our son, JUAN MIGUEL YBAÑEZ MAHUSAY,
and for all legitimate intents and purposes.
