AFFIDAVIT OF ONE AND THE SAME PERSON

I, RIZALINE PERFAS REBOTON, of legal age, Filipino, and presently residing at

615-D Aznar Rd., Riverside, Sambag 2, Cebu City, Philippines after having been duly
sworn to in accordance with law, depose and state that:

1. I am member of Social Security System (SSS);

2. As a member, I was issued a Unified Multi-Purpose ID card with No. CRN-

0111-6714272-9. In the said Identification card, my middle name was spelled
“PERPAS”. When I double checked with the SSS office, I was informed that
the name registered in their database is RIZALINE PERPAS REBOTON;

3. However, the correct spelling of my middle name is PERFAS, as shown in my

National Identification Card.1

4. Both the name Rizaline Perpas Reboton and Rizaline Perfas Reboton refer to
one and the same person, the herein Affiant;

5. This Affidavit is executed to attest to the truth and veracity of the foregoing
facts and for whatever legal purpose it may serve.
