AFFIDAVIT OF LOSS

I, ROGELIO NEIL P. ROQUE, Filipino, of legal age, married and a

resident of Bagontaas Valencia City after having been duly sworn to in
accordance with law, depose and state, that:

1. I am the possessor of two units of ICOM IC-V8VHF handset radio with

NTC Permit and License with desktop charger with serial numbers
0453888 and 0453886 owned by the Local Government Unit – City of
Valencia;

2. The above mentioned handset radio was lent by me to the Barangay

Officials of Valencia City;

3. Today, I discovered that the above mentioned handset radios can no

longer be located and further search proves futile;

4. I am executing this affidavit to attest to the truth of the foregoing

facts and to inform the General Services Office of the City of Valencia
of the same to facilitate the release of my accountability clearance.
