(CFR 30 .14000) Safety defects; examination, correction and records: Self-propelled mobile equipment to be used during a shift shall be
inspected by the equipment operator before being placed in operation on that shift.

Check off the applicable block for each item. Immediately report all damage and/or deficiencies to your supervisor.

Safety defects must be noted & corrected prior to equipment operation. Completed sheets must be submitted to the Department
Supervisor. Only trained personnel is allowed to operate any mobile equipment. Anyone operating mobile equipment without
completed and current checklist form is subject to disciplinary action. Documents must be maintained 1 year.
