IMMEDIATE ACTION
NO HOMEMADE APPLIANCES TO BE USED ON BOARD AMBREY SHIPS

INCIDENT - FIRE ON BOARD/FIRE IN CABIN


The Crewmember boiled water in his coffee mug with his homemade water heater
(heating element wired to a cable and plug). The crewmember left the cabin whilst
the heating element was on and his coffee mug fell onto the cabin table. The heating
element was then directly exposed to the wooden matt surface of the cabin table and
began to burn.
The incident was spotted when a passer-by noticed smoke coming out of the crewmembers’ cabin.
The fire alarm was activated, and a verbal announcement was made by crew. The instruction was
given to all POB to muster. Evacuation team started evacuating all passengers. The first fire team
were on standby with extinguishers and entered the space. The second fire team started boundary
cooling.
All passengers mustered and the fire team used a wet towel to cover the fire and opened all
portholes to provide ventilation. The fire team declared fire under control. To confirm all safe, the
second fire team entered the space and checked for heat in the next cabin and continue cooling the
adjacent areas. After this, the smoke cleared and the space became well ventilated. All passengers
were dismissed from the muster point once everything was under control and there was no sign of
heat in and around the space.

THIS NEGLIGENT INCIDENT DISPLAYS A VIOLATION OF ON-BOARD PROTOCOLS


WHERE IT IS NOT PERMITTED TO EAT, DRINK AND SMOKE IN CABINS.

IMMEDIATE ACTIONS
• UNDER NO CIRCUMSTANCES are homemade electrical appliances to be used on ANY
Ambrey vessel
• Electrical appliances are NOT to be left unattended while plugged into the vessel’s mains
electrical supply
FURTHER ACTIONS
All AMBREY ships will be furnished with a Portable Appliance Tester (PAT) and the C/Eng will
test and run a PAT register. All electrical appliances must be declared for testing when the
this becomes available on each vessel. NO NON-PAT tested equipment will be permitted
onboard once this is implemented.
INCIDENT THREAT AND LESSONS LEARNED
Fortunately, the incident was acted on quickly to avoid further damage to the ship and
ultimately risk to All Crew. If this had not been the case there could’ve been a far more
dangerous situation:
• Further damage to the adjacent cabins and the ship
• Harm to other members of crew
