ACTIVITY DOCUMENTATION REPORT

I.TITLE: Second Quarter Portfolio Day

II. LOCATION: Tabango National High School – SHS Campus, Tabango, Leyte (11-HUMSS
Einstein Classroom)
III. DATE: February 8, 2024
IV. INTRODUCTION
In pursuance to Regional Memorandum No. 1047, series 2023, entitled Conduct of
the Quarterly Portfolio Day for School Year 2023-2024, which announces the conduct of
Second Quarterly Portfolio on February 8, 2024, Tabango National High School promptly
responded to this announcement.
It has become a tradition of the school to call the attention of the parents for a
meeting both for the general assembly and homeroom basis. This is done to discuss with the
parents the important matters pertaining to the updates, as well as the performance and
accomplishments of the students.

V. ACCOMPLISHMENT
The Portfolio Day was facilitated by the section adviser, Jesamie B. Seriño of
TNHS with the following agenda:
1. Presentation of the newly-installed school head
2. Early Registration for grades 7&11
3. Updates of Homeroom PTA projects
4. Learners’ Performance Feedbacking/Signing of Report Cards
5. Video Presentation of TNHS Drum & Lyre Corps and Tambayayong
performance
6. Awarding of Certificates to academic achievers and students with perfect
attendance

VI. PROBLEM/S MET

- Some of the parents did not attend.
- Some of the parents were late.

VII. RECOMMENDATION/SUGGESTION
All parents must attend in the next HPTA meetings/portfolio days and come to school
on time.

VIII. GOOD PRACTICES LEARNED

The value of cooperation and support of parents to the learners helps the success of
the activity.

Continuous participation and support of parents for all school activities.

X. REPLICATION
Similar activities must be done to promote the welfare of the students and the school.
The support from the parents is badly needed to encourage the students to perform well in
their classes. This must be conveyed well to the parents in order for them to have an
unwavering support. The school, with the aid and initiative of the school head, must
encourage full support from all the teachers and parents to be cooperative in the said
activities like PTA meeting and implementation of projects. Parents together with the
teachers must help one another for the success of whatever plans the school may have for
the better future of the learners.

MINUTES OF THE MEETING

Type of Meeting: Second Quarter Portfolio Day

Location: Tabango National High School – SHS Campus, Tabango, Leyte (11-
HUMSS EINSTEIN Classroom)
Date: February 8, 2024
Time: 1:00 PM to 3:00 PM
Meeting Facilitator: Jesamie B. Seriño
Invitees: HUMSS 11-Einstein Learner’s Parents
Call to Order: The meeting was called to order
Attendees Present: 37 parents
Absent: 23 parents

Meeting Agenda:
1. Presentation of the newly-installed school head
The class adviser presented a short video to the parents about the installation
of the new school head, Sir Antonio B. Dacatimbang, Jr. In addition, it was
emphasized that there might be changes in the new administration for a holistic
management in improving the school.

2. Early Registration for grades 7 & 11

The class adviser presented the early enrollment schedule of Grades 7 & 11
starting February 6-8, 2024, where twenty-six (26) teachers were assigned to
nineteen (19) different schools in the municipality of Tabango.

3. Updates of Homeroom PTA projects

The class adviser presented to the parents the total amount of money
collected as of February 8, 2024 amounting to 3600 pesos The said project were
agreed on the previous meeting to procure wall fans/electric fans to have a well-
ventilated classroom. Hence, the collected money was then used to procure the
following:

. It was unanimously agreed by parents to procure a wall fan for the remaining
uncollected money.

4. Learners’ Performance Feedbacking/Signing of Report Cards

The class adviser discussed about the grades and the performance of the
learners. It has been discussed about students’ frequent absences and tardiness.
Parents were then advised by the class adviser to monitor their childs performance in
school or at home. Afterwards, the class adviser distributed the cards to the parents
of the learners.

5. Video Presentation of TNHS Drum & Lyre Corps and Tambayayong Performance
A video was presented to the parents of the previously held Tambayayong
Festival Activities which highlighted the performance of the TNHS students who won
1st Place in band competition and 2nd place in showdown competition.

6. Awarding of Certificates to academic achievers and students with perfect attendance

One of the highlights of the 2 nd Quarter Portfolio Day was the Awarding of
Certificates to academic achievers and students with perfect attendance. The class
adviser recognized the efforts of each academic achievers by giving certificates of
recognition together with the parents. Pictorial was then followed.

ADJOURNMENT: The meeting was adjourned at 3:00 in the afternoon after all the agenda
covered up as agreed upon.
