PHILIPPINES, after having been duly sworn in accordance with law, hereby depose and say:

That we are the true and Biological Parents of MARIA RASCHELLA

PASCUAL VELAYO and which was stated in her Birth Certificate;

That we are giving her the full financial support,

That we undertake the full responsibility of sponsoring and providing for her

medical insurance and living expenses.

That we are executing this affidavit to attest to the truth of the foregoing

and for her application for the issuance of her Korean Tourist visa.
