I am Gabrielle Anne B. Pagjunasan, a student of MS Pharmacology at the College of Medicine. I am

writing to request permission to use the Natural Products and Peptidomics Laboratory’s facilities and
equipment to conduct a portion of my experiments for my thesis entitled, “Anti-Leptospiral Activity of
Semi-Purified Fractions from the Ethanolic Leaf Extract of Psidium guajava (Bayabas) on Leptospira
interrogans serovar Manilae strain K64.”

My thesis adviser, Dr. Ailyn M. Yabes, has recommended that I conduct a portion of my thesis
experiments in collaboration with your laboratory. To facilitate this request, I kindly request the
Institutional Biosafety and Biosecurity Committee (IBBC) Form 2 of the Natural Products and
Peptidomics Laboratory as part of the certifications needed to conduct my study. I have attached a
copy of my thesis proposal and the IBBC Form 1 of my thesis for your perusal.
