MINUTES OF THE MEETING ON THE MONITORING AND EVALUATION OF

HOMEROOM GUIDANCE PROGRAM DELIVERY

I. CALL TO ORDER:

The Guidance Teacher Designate MAY LIEZLE D. BALDOS, Teacher III

with LUZVIMINDA R. ARATEA, Principal II convened at 1:30 pm for meeting for all the
teachers to discuss the monitoring and evaluation of Homeroom Guidance Program.

II. PRAYER:

The meeting started with a prayer led by RASSEL JOHN S. FOGATA,

Teacher I to ask for the blessings and guidance of our Lord on the implementation,
monitoring, and evaluation of the Homeroom Guidance Program.

III. ROLL CALL

The Chairperson called the attention of teachers to be acknowledged.

IV. PRESENTATION OF THE AGENDA:

The principal presented the matters to be discussed and to be done.

1. Status of Homeroom Guidance Program.
2. Results of monitoring for teachers and pupils.
3. Issues and concerns encountered during the implementation.

4. Recommendation for possible intervention/remediations for the

improvement of the program.

V. MEETING ORDER

Mrs. Baldos discussed the status of the implementation of the Homeroom

Guidance Program by grade level. Monitoring was done every end of the quarter
wherein the learners’ progress outcome was given emphasis. There were some issues
that need to be addressed during the monitoring and evaluation. Below are some
issues encountered during monitoring and evaluation;
1. Upon retrieval of modules, there were some pupils returned
their modules without any answers.
2. Some pupils did not meet the deadline of submission for
retrieval.
3. Some did return the modules.
In connection to the issues encountered, immediate interventions or remedials
were crafted to address the problem. Through the recommendation of Mrs. Aratea,
Principal II a SLAC session was made to brainstorm for the formulation of the
remediation activity to resolve the problems encountered for the improvement of the
program.

VI. CLOSING MESSAGE:

The closing remarks was given by Elementary School Principal I, Ma’am

Luzviminda R. Aratea commending all the teachers for a job well done.

VIII. ADJOURMENT:

The meeting was adjourned at 4:00 in the afternoon.
