13% - LILIAN 20% - PATRICK ( 50%) = 30% COMPANY BANK

PETTY CASH ( 3000 - 500 CASH )= 2500 ( MODAL )

WORKER AGREEMENT
1. Probation is given 1 month / 30 days .

2. No EPF deduction for the probation month.

3. Salary is as stated below , basic + commission + full attendance . Worker have to

understand and well agreed with the term below to avoid any misunderstanding.

4. Salary will be given 14 days - once ; twice a month . Worker can choose in cash or

bank in term .

5. No commission is given is attendance is not full ( 12day/12day).

6. Medical claims is only for accidents at workplace .

7. Any leave / emergency leave must be told 24 hours before OR double deduct (one

day salary) for the day.

8. Sick-leave can be apply with the proof .

9. Must wearing : shoes and long pants with ; cap - covering hair .
