Functional location: PP1-C501 (Steamer off gas compressor)

Schedule: 25.03.2024 ~ 15.03.2024

Work scope: Lump sum basis to remove, replace, alignment and perform PM Job

Work scopes:

1. Lump sum basis to supply manpower, tools, consumables etc.

2. Prepare an inspection checklist to record the inspection result.

3. Perform pre-checking of the coupling alignment before removing gearbox.

4. Replace new gearbox.

5. Perform laser alignment (motor to gearbox and gearbox to compressor). Supply stainless steel
metal shims. Maximum of 5 shims at each anchor of the pump. Maximum misalignment is 0.05mm
(total indicated runout).

6. Must inform LCTM to witness the alignment result. Else, LCTM has the right to request contractor
to redo the alignment.

7. Perform PM Job.

8. Standby during pump commissioning and attend to site if abnormalities were found.

Note:

• Replace gearbox base bolt nut c/w spring washer.

To engage contractor to do replace seal pressurization piston repair kit.

Work scopes:

1. Remove and install seal pressurization piston at site.

2. All "o" rings and seal to be replace new.

3. All other metal parts to be check & clean when necessary

4. Assemble and perform pressure test.

5. To submit work report (complete with picture) showing repair & replace for the parts of the seal
pressurization piston.

6. To return back the damaged parts which already been replaced.

Remark:

LCTM will provide repair kit, crane and scaffolding.

Work scope: Overhaul agitator

Agitator details

Model/type: AV 37-1450

Schedule: 15.03.2024 ~ 15.04.2024

Work scope: Lump sum basis to remove, overhaul, and reinstall the pump. See work scopes for
details.

Work scopes:

1. Lump sum basis to supply manpower, tools, consumables etc.

2. Prepare an inspection checklist to record the inspection result.

3. Perform pre-checking of the coupling alignment before agitator.

4. Lift down agitator and bring to workshop for overhaul.

5. Disassemble the agitator, clean and perform inspection on the pump bearing housing, bearing
journal and mechanical seal.

6. If the housing, journal are damaged, inform LCTM to send for repair.

7. Upon receiving the repaired agitator shaft and housing, make sure to inspect and confirm the
measurements fulfill the requirements. Otherwise, inform LCTM to resend the items for repair.

8. Change the mechanical seal, bearings, gaskets and O-rings.

9. For all clearances, inform LCTM for confirmation prior to assembly.

10. Make sure to fill in the checklist during assembly and installation.

11. Perform pressure test, witness by LCTM personnel and Install agitator at site.

of the pump. Maximum misalignment is 0.05mm (total indicated runout).

13. Must inform LCTM to witness the alignment result. Else, LCTM has the right to request
contractor to redo the alignment.

14. Standby during pump commissioning and attend to site if abnormalities were found.

Notes:

1. All spare parts and crane to be supplied by LCTM

2. All logistic, coordination, transportation, reporting and services required to complete the job
is under contractor’s scope of work, unless mentioned to be supplied or provided by LCTM in the
PR.

3 NOTE: The CONTRACTOR must submit to the COMPANY, the "Job Completion Report# for the
WORK performed within one (1) month from completion of the WORK failing which the
COMPANY may withhold payment until such time the CONTRACTOR submits the Job Completion
Report. The time for the performance of the WORK shall be of the essence of the

CONTRACT. The CONTRACTOR shall perform the WORK by the scheduled date

stated failing which the COMPANY may terminate the CONTRACT without any

liability whatsoever. Without prejudice to any other rights or remedies

which the COMPANY has against the CONTRACTOR, the COMPANY shall be

entitled to charge a sum equivalent to 0.3% per day and up to the

maximum of 10% of the amount payable for the WORK as liquidated damages

and not as penalty for any delays. The CONTRACTOR shall pay the

liquidated damages to the COMPANY within thirty (30) days of a notice

from the COMPANY failing which the COMPANY may offset against any

amounts due to the CONTRACTOR."
