To Whom It May Concern,

I, Ma. Lourdes C. Adocon, the undersigned, am the legal guardian of Trisha Mae C. Adocon, 21,
who is a 4th-year student at the Pamantasan ng Lungsod ng Valenzuela, currently undertaking
an internship at the Philippine Information Agency (PIA) under the supervision of the Program
Management Division.

I hereby grant permission for Trisha Mae C. Adocon to attend events outside of the Philippine
Information Agency (PIA) premises in relation to their internship activities. I understand that
these events are related to the internship program and will be conducted under the supervision
and guidance of the Program Management Division.

I understand that these events are considered valuable learning experiences and are integral to
Trisha Mae's professional development.

I release the Pamantasan ng Lungsod ng Valenzuela, the Philippine Information Agency (PIA),
and the Program Management Division from any liability or responsibility for any unforeseen
circumstances that may arise during the participation of Trisha Mae C. Adocon in these events.

Furthermore, I understand that Trisha Mae's participation in these events is in accordance with
the approval of Dean Michville Rivera of the Pamantasan ng Lungsod ng Valenzuela.
