SUBJECT Amendment to Department Memorandum No. 2023-0071 dated

January 16, 2023 entitled “Guidelines on the Updated FHSIS
Manual of Procedures (MOP) version 2018”

The Department Memorandum No. 2023-0071 dated January 16, 2023 entitled “Guidelines on the
Updated FHSIS MOP version 2018” is
hereby amended to change the effectivity from CY 2023
to CY 2024 except for the following revisions and inclusions:

1. Revision of Basic Oral Health Care (BOHC) for Senior Citizens;

2. Removal of the word “newly” in
the following indicators for patients with hypertension
and diabetes risk assessed using the PhilPEN;
3. Inclusion of “Completed 90 tablets of Iron with Folic Acid” in Postpartum;
4, Inclusion of “Date of Completion” in Micronutrient Powder 6-11 months and 12-23
months
5. Reporting of BOHC — the BOHC per client shall be reported “once” within the year or
annually in relation to the eligible population with the denominator also being measured
annually;
Inclusion of Legend in Individual Patient Treatment Record;

Removal of EBF column with 1 %, 2 %, 3 %, 4-5 months in TCL;
Removal of “as needed” in
provision of BOHC Pregnant Women; for
we Removal of
“hilot” in TCL Birth Attendant; and

 10. Removal of Socioeconomic Status (National Household Targeting System and Non-
NHTS) in all reporting forms.
All other provisions stipulated in DM No. 2023-0071 dated January 16, 2023 not affected by this
amendment still stand in effect.
