Subject: GUIDELINES ON THE CONDUCT OF LIMITED FACE-TO-FACE


CLASSES FOR AY 2022-2023

Pursuant to CHED-DOH Joint Memorandum Circular No. 2021-004, CMO No. 1, s. 2022
CMO No. 05 s. 2022, and the recent public pronouncement of CHED last August 29, 2022 in
relation to the gradual reopening of campuses of Higher Education Institutions for limited
face-to-face classes, the following guidelines shall be implemented in the University effective
Academic Year 2022-2023:

I. GENERAL GUIDELINES

1. To ensure the health and safety of the entire PLM community, strict compliance with
the health protocols and guidelines provided herein is expected from all students,
faculty members, non-teaching staff and other University stakeholders. Any non­
compliance or violation shall be dealt with in accordance with available disciplinary
or other measures.

2. Being a public university, PLM will continue to strictly implement the relevant and
latest issuances of the IATF, Department of Health, CHED, the Civil Service
Commission and other relevant government authorities in relation to the conduct of
face-to-face classes.
3. In accordance with the latest public pronouncement of CHED during its meeting with
HEIs last August 29, 2022, all students, whether fully vaccinated or not, may be
allowed to attend face-to-face classes within the University premises.

4. However, students from the Colleges of Medicine, Nursing and Physical Therapy
with hospital and/or community rotation/exposure are considered as frontliners and
are therefore required to be fully vaccinated. Booster doses will be required if
deemed necessary by their respective training facilities/ affiliation centers.

II. HEALTH PROTOCOLS REQUIREMENTS

A. ENTRY TO THE UNIVERSITY

1. To avoid congestion and maintain physical distancing within the campus premises, only
students scheduled to attend face-to-face classes, or those with approved academic or
extracurricular activity, library visit, faculty consultation or other official business, will
be allowed to enter the University.1 For this purpose, students should present their
enrollment slip or proof of appointment prior to their entry to the University.

2. Ingress and egress of students, teaching and non-teaching personnel shall be managed by
the University Security Office (USO) to avoid congestion of people. This includes
prescribing pathways for entry and exit to specific buildings and offices.

3. Entry of visitors (including but not limited to alumni, suppliers/contractors, and parents)
is allowed but shall strictly be by appointment only. No appointment, no entry. The
University Security Office (USO) shall adopt a color-coded identification scheme for
those with specific appointments. Appointment slips must be secured by the visitor at the
USO upon entry to PLM, which must be signed by the office or college concerned,
indicating the date and time of visit. The slip will be checked by the USO upon exit
prior to the return of the color-coded ID.

4. Students and visitors entering PLM must accomplish the STAYSAFE.APP, a contract
tracing application, prior to entry. The University Health Service (UHS) will review the
StaySafe App and notify students and visitors who are “NOT ALLOWED” under the
app to verify their health condition.

5. Teaching and non-teaching personnel who will enter PLM must accomplish the
ONLINE HEALTH DECLARATION FORM by 3:00 PM a day before their entry to the
University. The health declaration forms will be reviewed daily by the University Health
Service which, in turn, will determine whether the personnel will be cleared for entry to
PLM or not.

6. Mandatory temperature screening prior to entry will be done by the USO. Those with
body temperature of more than 37.5°C will not be allowed entry to PLM and will be
immediately referred to the UHS for proper management.

7. Likewise, persons who exhibit signs and symptoms of COVID-19 (such as, but not
limited to, fever, cough, colds, difficulty of breathing, sore throat, headache, body
pain/myalgia, loss of taste and/or smell, diarrhea) will not be allowed entry to PLM and
will be referred to the UHS for proper management.

8. Students, teaching and non-teaching personnel, and visitors are required to wear medical
grade face masks, preferably KN95, or surgical masks which could cover the nose and
mouth. Cloth masks are not allowed as these do not protect against viruses.2

9. All are encouraged to bring their own hygiene kits consisting of at least 70% isopropyl
alcohol, soap for handwashing, and tissue or face towel.3 Sharing of hygiene kits is
strongly discouraged.

10. In the event a student, faculty member, staff or visitor develop or manifest any sign or
symptom of COVID-19 (such as, but not limited to, fever, dry cough, colds, difficulty of
breathing, sore throat, headache, body pain/myalgia, loss of taste and/or smell, diarrhea)
they should be reported to the UHS immediately for proper medical management. PLM
has a designated isolation room for symptomatic persons.

11. Faculty members (as far as the students inside their classes are concerned) and heads of
colleges/ offices (as far as their faculty, staff and visitors are concerned) are responsible
for reporting of symptomatic persons to the UHS immediately.

12. Those who were sick or with symptoms of COVID-19 within the last five (5) days prior
to entry to the University are required to present a medical certificate from his/her
primary physician or local government unit to the University Health Services (UHS) for
re-entry clearance.

B. CONDUCT INSIDE THE UNIVERSITY

Upon entry and throughout their stay within the University premises, students, faculty
members, staff and visitors should always observe the following:

a. Wearing of medical-grade face mask.


b. Following the posted signages and foot traffic directions provided.
c. Maintaining strictly the prescribed physical distancing.
d. Constantly sanitizing hands with 70% isopropyl alcohol before and after entering the
office/cl assroom/laboratory.
e. Bringing one’s own food or “baon”, drinking bottles and utensils. Sharing is not
allowed.
f. Eating ONLY at designated areas (University Activity Center and gazebos).
g. Placing “infectious wastes” like disposable masks, gloves, or soiled tissues, used
disposable caps and utensils in a sealed plastic bag before disposal in the designated
hazardous waste trash bin.
h. Waiting for one’s cue in using the comfort room. Each CR has a designated number
or people allowed at any given time. Observe handwashing protocols after using the
CR.
i. Staying inside one’s office or assigned classroom during office hours or classes.

j. Leaving the office, classroom/laboratory or university premises, immediately after


one’s duty or class.

Violation of any of the above shall render the student liable for disciplinary action
pursuant to the applicable provisions in the Student Manual and/or interim guidelines
from OSDS, and with respect to the teaching and non-teaching personnel, the applicable
CSC rules and regulations.

C. CONTINGENCY PROTOCOL IN CASES OF SUSPECTED CASES

1. IF/WHEN a student/teaching and non-teaching personnel becomes symptomatic


during limited face-to-face classes:

Those who become symptomatic will be referred to the University Health Services
and brought to the Isolation Room for evaluation, screening, and treatment, as may be
warranted. Referral for RTPCR testing will be given to determine COVID-19
infection.

a. If student, teaching, and non-teaching personnel tested positive, he/she


will be immediately endorsed for home isolation, as may be ascertained
by the UHS and/or the attending medical doctor.

b. After recovery and completion of the prescribed quarantine period as may


be applicable, the student will be required to submit medical clearance
from UHS before he/she is allowed to resume with his/her face-to-face
classes.

2. IF/WHEN student, faculty, employee is exposed in campus to an individual who


tested positive:

a. The student, faculty, employee will be required to undergo RT-PCR


testing and shall quarantine for at least 7-days from the date of last
exposure, if fully vaccinated, or at least 14 days if partially vaccinated or
unvaccinated.4

b. If the swab test reveals a positive result but he/she remains asymptomatic,
he/she will be required to continue isolation for 7 days from the sample
collection date.5

D. REPORTING OF COVID-19 CASES

1. Strict surveillance shall be conducted by the UHS and Presidential Committee on


Health to monitor transmission of COVID-19, identify cases and their close contacts.

2. All heads of units and deans shall immediately report cases of COVID-19 in their
respective offices/colleges to the UHS and the Presidential Committee on Health.

All other orders, memoranda and issuances that are inconsistent with this order shall
be deemed repealed, superseded, or modified accordingly.

This order shall take effect immediately.
