1 The work of labour inspection is if the workers have made any complaint, to investigate
the situation.

To issue a notice to a company when it sees it is not following health and safety.

the action which labour inspector could take following a visit are given below
 Labour inspector can issue warning letter if he sees any fault on the workplace.

Task 2: what are the positive indicators of health and safety culture in the organization?

There are many positive indicators of health and safety culture in the organization which are given
below:

1 This organization has been working for a long time and workers in this organization are skilled in
their work and these skilled workers enable the new (contract basis) workers to do the right
thing.
2 In the production and storage areas experienced workers are attached with new temporary
workers in a “buddy system”. This enable the new worker to learn correct learning method
about their job and enable the new temporary to ask question ask question about their work
from skilled worker.
3 According to scenario there is a great reliance on the knowledge held by many long serving
workers all of them are highly proficient in their role.
4 There is found communication between long serving workers and temporary workers.
5 This organization operates in two shift for each shift 8 hours are given approximately which is
proper set up of working hours. This organization care for the welfare of their employees by
given 8 hours for work which is suitable time for to balance their work.
6 The site managers walk around the factories on daily basis he interacts with the workers and
listen to their concerns people work safely due to the site manager.

while cleaning the electric milling machine?

1 The temporary young worker who has only been the organization for a week is seen to be
enthusiastic and fast at his work.
2 The young worker is so excited about his work that starts cleaning the electric milling machine
alone without buddy system (in which attach an experienced worker to a new worker for the
assistance of new worker).
3 The young worker did not clean the electric milling ever before and nor he has seen or analyzing
anyone doing this work. Due to his he does not about the precautions that he should while
cleaning the machine as he is new on the job.
4 The worker has low experience about “risk perception” due to which he the task of cleaning the
electric milling is easy work that is why he begins the cleaning of milling machine alone without
the help of experienced worker.
5 The worker is very excited to his work and wants to do more as soon as possible therefore he
starts the work without any supervision.
6 The young worker thinks he can help his co-workers by cleaning the milling machine early.
7 The temporary young worker thinks his starting to work at first will make positive impression on
others.
8 The young worker did not know how to work safely on the workplace because he starts the
work without the isolating the power and stands on a chair to gain access to the machine and
lean over the side of large funnel structure and try to keep balance on chair during this with the
help of his knee, the on button of milling is pressed.
9 The young worker did not used any PPE instead of this he puts both hands inside the machine to
remove the spike drum.
10 As the young worker was temporary and new in the organization therefore he was not how to
manage health and safety at the workplace and organization did not provide health and safety
training.
11 The young worker was not mature about his work because his age was17 which is not suitable
for any risky task.
12 The young worker was also excited when the manager announced that he would give him an
extra bonus for this cleaning task and the start the without any assistance regarding to health
and safety.

procedures across the organization?

There are many possible reason for a general lack of knowledge about the emergency procedures across
the organization which are given below.

As in the organization accident rate is low that is why no preparation have been made to deal in
emergency situation.

1 When the accident happened in the storage room all the workers run towards the
injured for but no one knows how to deal in this emergency situation these things
shows general lack of knowledge about the emergency across the organization.
2 There is lack of detailed emergency response procedures in the organization.
3 No training was provided to workers on individual roles and responsibilities prior to
accident.
4 No emergency stop indication is displayed on the workplace bur according to scenario
fortunately a worker was successful to press the emergency stopper button.
5 There is no planning and procedures for the arrangements during the emergency in
the organization.
6 In the emergency situation no firs aid and medical assistant was provided to injured
worker.
7 First aid training was not provided to worker because when the accident happened no
one was ready for giving the first to injured worker.
8 First aid kit was not provided across the organization for managing the emergency
situation.
9 workers were given written information about emergency situation and no training was
not provided how to act in emergency situation.
10 Organization did not did not displayed emergency contact number on the workplace as
when accident happened workers to the office for help and one office workers called
for ambulance.
11 Organization did not provide well trained first aider at the workplace.
All these reason shows general lack of knowledge about the emergency procedure
across the organization.
Task 6: worker’s responsibilities in the workplace

The young worker decided to clean the electric milling machine alone without the assistant of buddy. In
this way he contravened the “buddy system”.

1. The young worker contravened the article 19 (a) of C155 safety and health as he did not
cooperate in the fulfillment with the employer obligations. As the young worker decided to

Task 7: how a permit to work system works in practice?

1. PTW should be issue only by the competent person who has the authority for issuing the permit
and should be receive only by the competent who will work on it.
2. PTW must be signed by the authorizer who issue the permit and receiver who receive the
permit before issuing the permit.
3. Because permit is taken for high risk activity therefore risk related to work should be clearly on
PTW.
4. Date of starting the work and completing the work must be mention on PTW.
5. Period of work should be fixed on PTW and extension should be only with the premises of
authority who issue the permit.
6. Only competent worker should be hire for the specified work for which permit is taken.
7. All work described in the PTW should be supervised by the competent person.
8. As permit to work is taken for risky activities therefore it is emergency and first aid facilities
should be available at the workplace.
9. In permit to work must be identify which PPE should be used for the task.
10. Control measures must be mention in PPT.
11. Identification all the hazards to be carried out.

Task 8: why does this organization need urgently review its health and safety management system?

1 New workers had been hired in the organization to meet the lack of production, which
were not given the training about health and safety.
2 As the scenario describes there is increasing production rate to ensure order fulfilment
new risks are create with different business strategies therefore, it is necessary to
review the health and safety management system.
3 The review will reassure the workers that the organization is concerned about their
safety and promotes worker’s morale.
4 Many flaws have been in this organization as the scenario describes “on the of accident
all the workers run to the injured worker for help but no one was not how to help the
injured worker” all these things shows that the workers were not given the training of
first.
5 Organization did not first aid kit on the workplace and not emergency contact was
displayed on the workplace therefore It is to review the health and safety management
system.
6 When there happen a near miss in the storage room the focused to cleanness instead of
investing the near miss.
7 Because the accident happened in storage room during the cleaning, therefore it is
necessary to review.
