    OF PLANT ACCIDENT TO BE TAKEN THE FOLLOWING TO BE COMPLETED

     Record down all the details of the incident, Minor Near Miss; Major Near Miss
    NEAR MISS

    Any incident which has the potential


    INCIDENT

    to ;  Secure the area and ensure the area is safe,  Immediate Supervisor.  Preliminary Accident Notification Form
     Cause harm or danger,  Switch off and lockout Machinery  MVA Insurance Claim Form,
    involved, Major Near Miss;  Motor Vehicle Accident Investigation Report
     Cause property damage,
     Ensure that no further injury/damage can  Project HSE Manager  Sect 12.04
     Cause environmental damage,
    take place,  Site Agent / Manager.
     Cause a major incident.
     Take necessary photographs.

     Any Minor incident,  Record down all the details of the incident,  Site Agent / Manager,  Preliminary Accident Notification Form
      
    ACCIDENT

     Small bumper bashing, Secure the area and ensure the area is safe, Project HSE Manager MVA Insurance Claim Form,
     Switch off and lockout Machinery  Office Insurance Dept  Motor Vehicle Accident Investigation Report
    MINOR

     Repair costs below N$ 10 000.00,


     Windscreen repairs
    involved, within 12 hours,  Sect 12.04
     Ensure that no further injury/damage can  Local Police if on Public
     No Third Party involvement,
    take place, Road,
     No Injuries sustained.
     Take necessary photographs,  Plant Dept if repair work is
     Take down details of possible witnesses. needed.

     Any Major Incident,  Record down all the details of the incident,  Site Agent / Manager,  Preliminary Accident Notification Form
    ACCIDENT

     Repair costs above N$ 10 000.00  Secure the area and ensure the area is safe,  Project HSE Manager  MVA Insurance Claim Form
    MAJOR

     All Third Party involvement,  Switch off and lockout Machinery  Office Insurance Dept  Motor Vehicle Accident Investigation Report
     All Accidents involving Injuries,
    involved, within12 hours,  Sect 12.04
     Ensure that no further injury/damage can  Local Police if on Public
     All accidents involving Fatalities.
