Scheduled Caste Certificate (Service)
INSTRUCTIONS & DOCUMENTS REQUIRED:
1. Identity/Citizenship Proof (Any one of the following):
1. Driving License, 2. Pan Card, 3. Aadhaar Card

2. Resident Proof/Address Proof/Ownership Proof (Any one of the following):

1. Passport, 2. Voter ID, 3. If Govt. Employee Office ID Card with same address,
4. Current Electricity Bill of Applicant Name.

3. Pre-verification report
For Rural Area:
Pre-verification report by Sarpanch/Nambardar and Patwari.
For Urban Area:
Pre-verification report of Ward Member & E O(Estate Officer) / ZTO (Zonal
Taxation Officer) / Secretary of Municipal Corporation.
4. Being a permanent resident of the state of Haryana is mandatory. If the applicant is
not living at the present address from birth, then the applicant should attach the cast
report received from the concerned Sarapanch/Nambardar, Patwari, Tehsildar from the
native residence of Haryana.
5. The scanned photo of whose certificate is to be made will not be valid.
6. If the woman is married, in that case the caste report of her father received from
Sarpanch/Nambardar, Patwarai, Tehsildar in case of rural area and report from
councilor, municipal corporation/municipal committee in case of urban is mandatory.

7. If the girl’s certificate has to be made, then the report of the girl’s married and
unmarried is mandatory.
8. If the child is born in the state of Haryana but his father is not a native of the state of
Haryana, in that case the child’s father’s caste certificate issued from the original state
or verified caste report from the parent State.
