CLOSE-OUT REPORT: MISSING ROLLER COMPACTOR

1. Executive Summary:

During a routine survey conducted on 23.10.2023, a missing roller compactor was discovered by the
Incharge foreman Mr. Wahid at the Sajco working area, this close-out report outlines the details of the
incident, actions taken to resolve the situation, and recommendations for preventing similar occurrences
in the future.

2. Incident Details:

On 09.10.2023, the Incharge foreman identified the absence of a roller compactor, which was assigned to
the project site for the Noman contractor, The missing equipment was last recorded as being in use on
04.10.2023 and was subsequently found to be unaccounted for during the routine inspection.

3. Actions Taken:

Upon discovering the missing roller compactor, the following actions were promptly taken:

a. Notification: The Incharge foreman immediately reported the incident to CM/department head / Safety

b. Investigation: An investigation team was formed to determine the circumstances surrounding the
missing equipment. The team conducted interviews with the project personnel to gather relevant
information.

e. Security Measures: Strengthened security measures were implemented to prevent future equipment
theft or misplacement and implement stricter check-in/check-out procedures.

4. Recovery and Resolution:

After diligent investigations and search efforts, the missing roller compactor was recovered on 23.10.2023.
It was found abandoned in an unauthorized location in the Sajco working area away from the project site
by the foreman Incharge Mr. Wahid, The roller compactor was inspected for damages and was found to
be in working condition.

5. Recommendations:

To prevent similar incidents in the future, the following recommendations are suggested:

a. Enhanced Security Measures: Continuously evaluate and improve security measures.

b. Regular Equipment Audits: Conduct periodic, comprehensive audits of the inventory to maintain
accurate records and promptly identify any missing or misplaced items.
c. Employee Awareness and Training: Provide training to all project personnel regarding equipment
handling, accountability, and the importance of reporting any incidents of missing or stolen equipment
promptly.
