Format for Making a Module

Title Page

- Contain the title of the module, name of the School, the Author and the year produced.

Acknowledgement

- The authors personal message

Table of Content

- The summary or the content of the module from the first lesson up to the end.

General Direction

- the common guideline or procedures in using the module

Pre-test

- Compose of sample questions/ examination from the entire module that will measure the
learning capacity or level of knowledge of the students that will use the module.

Learning Content/ Lessons

- Include Introduction page containing general description of the module, general objectives and

the general learning outcomes

- Compilation of lessons/ topics to be discussed or covered to accomplish the entire module. See

the format for each lesson/ topics.

Post-test

- This is the final examination that covers all the lesson/ topics of the module. It is a random
question/ exam taken from each lesson.

Key to Correction

- The compilation of answer from pre-test, every activities of the entire lessons and the post-
test.

bonded)

- Short description of the learning outcome of the students after taking this lesson.
