History

A 28-year-old, G1P1 (1001) presents with persistent localized breast pain and swelling in the
right breast. She had a cesarean section 3 weeks prior to consult and was recovering well until 3
days ago when she reports initially experiencing flu-like symptoms and redness over the breast,
which have progressed despite self-management measures. She has been unable to keep herself
warm despite several blankets and has reduced appetite, nausea, vomiting and lethargy.
She is breast-feeding and has had very sore nipples since the birth but feels this is normal and has
been using petroleum jelly to soothe them.
She no longer has vaginal bleeding but has a minimal brown discharge with an odor which she
says is improving. Bowel habit is normal and she has no urinary symptoms.

Questions:
1. What are the differential diagnoses?
2. What further diagnostic tests will you request?
3. How will you manage and treat this patient?
4. How will you counsel/instruct this patient with regards to her condition?
